Dental Care CoordinatorCrescent Community Health - Dubuque, IA 52001Summary: The Dental Patient Care Coordinator is responsible for the management of the Delta Wellness Program, (DWP) and Universal Data System, (UDS) Dental Measures. The Coordinator works with the Dental Clinic Manager, the QI Manager and collaborative teams to organize and lead the DWP requirements for compliance.Essential Duties: Essential job duties for this position include the following items. Other duties must be performed as assigned or required.Assist the Dental Clinic Manager with establishment of DWP program goals and objectives.Assist dental team(s) in development of DWP program, key measures, criteria and ongoing feedback.Assist in implementing team-directed improvements by using and teaching others the DWP model.Facilitate daily morning huddle with the dental team to review patient census for the day and strategize how to maintain compliance with DWP requirements.Complete on-line dental risk assessments for patients using PreViser.Develop and conduct training for staff and patients including training on PreViser,Contact for CCHC collaborative communications, including submission of required reports (as defined by dental manager).Provide patient education, guidance and awareness of chronic disease to empower patients with self-management of their dental health and how it relates to their systemic health.Act as advocate and liaison between patients and the dental practice to maximize effectiveness of dental care.Collaborate with finance department to ensure that patients receive financial counseling for assistance in planning for billing copays and that dental procedures are properly coded for billing to insurance companies.Facilitate referrals for dental treatment that are beyond the scope of our practice as recommended by referring dentist.Monitor patient dental database and produce reports as needed or requested.Using current software, such as Dentrix and Centricity, track and analysis data and report key measures/trending of both the DWP and the UDS Sealant measures monthly.Attend and actively participate in all department and agency-wide meetings.Serve as a member of the Performance Improvement Committee and provide updates to the committee on DWP compliance.Assist in orienting new dental employees to DWP program.Maintain and follow all CCHC infection control, safety and compliance requirements according to OSHA, HRSA, HIPAA and other state and federal clinical quality guidelines.Plan and assist with public relations activities such as health fairs.Perform prior authorizations for DWP, Medicaid Insurances, as well as private insurances.Professional representation of the clinic to external agencies as directed by Dental Program Manager.Availability for occasional local travel and infrequent travel outside of the local area.Work toward continuous quality improvement.Stay current with changing best practices and technologies.Uphold, support, and promote all company policies and procedures.Qualifications: To perform this job successfully, the individual in this position must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills and ability required.Education/Experience:A minimum of 2 years' work experience as a dental receptionist, assistant and/ or hygienist, 5 years preferred.Demonstrated knowledge of Microsoft Office required. Experience with insurance billing and prior authorizations highly preferred.Highly desired for dental experience working with high risk adolescent, adult patients with asthma, diabetes, cardiovascular abnormalities, and/or obesity.Highly desired for experience using Dentrix, Centricity and PreViser software programs.Language Skills:Communicate clearly, professionally, and respectfully with all patients and staff.Mathematical Skills:Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.Reasoning Ability:Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems.Ability to deal with nonverbal symbolism (formulas, scientific equations, graphs, musical notes, etc.,).Ability to deal with a variety of abstract and concrete variables.Certificates, Licenses, Registrations:If certified, completion of credentialing and privileging requirements.
