Youth RHY Social Worker — Counseling & Case Mgmt

Rising Ground in Brooklyn, NY is seeking a Full Time Social Worker to support RHY program participants. Under the supervision of the Lead Social Worker, you will conduct intakes, mental health assessments, ISP development, and provide clinical counseling to youth. You will ensure documentation, housing applications, and coordination with providers to track services and prepare reports. 40 hours weekly with growth opportunities within the program. #J-18808-Ljbffr Rising Ground
