Responsibilities
- Conduct comprehensive PT evaluations and develop individualized treatment plans for residents with a range of conditions affecting mobility, balance, gait, strength, and functional independence.
- Implement evidence-informed therapeutic interventions and modify plans based on ongoing assessment and progress.
- Document evaluations, treatment plans, progress notes, and outcomes in the resident’s medical record; communicate changes to the interdisciplinary team.
- Collaborate with nursing, therapy assistants, physicians, and other care members to coordinate goals and discharge planning.
- Educate residents and families on home exercise programs, safety strategies, and activities to support continued recovery and function.
- Ensure adherence to infection control, safety, and regulatory standards within the SNF setting.

Requirements
- Active, unrestricted Physical Therapist license in New Hampshire or eligibility to obtain licensure.
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Current CPR/BLS certification.
- 1+ year of PT experience in a skilled nursing facility or long-term care setting preferred.
- Ability to work in a travel assignment for 13 weeks and to collaborate effectively with a multidisciplinary team.
