Key Responsibilities Conduct thorough evaluations of patients to assess their physical condition, utilizing a variety of diagnostic tools to determine the extent of disabilities or dysfunctions. Design and implement personalized treatment plans that align with the individual goals of patients based on their age, medical history, and specific health issues. Document patient care thoroughly, ensuring compliance with regulatory standards and insurance requirements while maintaining accurate, up-to-date medical records through our Electronic Medical Records (EMR) system. Communicate effectively with patients and their families regarding treatment progress, offering guidance and education to facilitate recovery at home. Collaborate with multidisciplinary teams, including physicians and occupational therapists, to coordinate care and optimize patient outcomes. Maintain a safe, clean workspace, and regularly check therapy equipment, ensuring everything is functioning properly to guarantee patient safety. Provide support in training staff and developing educational materials pertaining to treatment modalities and evidence-based practices. Patient Population / Client Focus At Revitalize PT and Fitness Clinic, you will work with a diverse range of clients, primarily focusing on those recovering from orthopedic surgeries, sports injuries, and chronic pain conditions. Your expertise will not only help restore mobility but will also inspire confidence in our patients as they work towards regaining independence and improving their overall quality of life. Qualifications A valid Physical Therapy license in good standing within the state of California. A National Provider Identifier (NPI) for billing and insurance purposes. A Bachelor's Degree in Physical Therapy or a related field from an accredited program. Preferred Experience At least 2 years of professional experience in physical therapy, with a focus on orthopedic or sports rehabilitation. Demonstrated ability to develop tailored rehabilitation programs based on evidence-based practices, suited for patients with varying levels of ability and comorbidities. Strong communication skills, both verbal and written, to effectively engage with patients and their families. Experience collaborating in a multidisciplinary healthcare team to enhance patient-centered care. Compensation and Benefits We offer a competitive salary ranging from $100,000 to $125,000 annually , commensurate with experience, along with a comprehensive benefits package.
