CASE MANAGER

Job Summary Case Manager at Yosemite Treatment Center in San Andreas, California. The role involves administering, developing, and facilitating case plan completion for youth in a short‑term residential therapeutic program that emphasizes strength‑based and trauma‑responsive practices. Location 10400 Fricot City Road, San Andreas, CA 95249, USA. Compensation and Benefits Pay : Starting at $24.00 per hour. Benefits : Medical insurance eligible after 60 days of full‑time employment. Full Rite of Passage benefit package available after 90 days. 403(b) matching up to 6% after 12 months. Paid time off accrues from the first day of service. Responsibilities Administer, develop, and oversee the completion of case plans and treatment programs. Develop and manage the MAP Plan. Provide monthly progress notes and weekly student updates. Facilitate weekly group meetings. Conduct all duties with respect, consideration, encouragement, and positive guidance to support youths' overall success. Qualifications Be empathetic, patient, consistent, compassionate, and possess an honest work ethic. Desire to work with troubled teens. Bachelor’s degree in a related field (BA/BS). At least 3 years of case management experience with youth or juvenile placement agencies. Minimum age 21 years. Pass a criminal background check, drug screen, physical, TB test, and child abuse registry search. Working Hours Sunday – Thursday or Tuesday – Saturday; 8‑hour shift with 1‑hour break.
