Description

The Health Assistant (HA) works as a member of a multi-disciplinary primary care team providing integrated primary care services to homeless and low-income patients seen at Old Town Clinic. The Care Team is responsible for proactively managing the healthcare needs of the patients on their assigned panel, including behavioral and medical health needs. The HA is responsible for providing support to the Care Team Managers ( CT M) and providers on each primary care team.

Program: Old Town Clinicis a health clinic providing treatment for acute and chronic health concerns, primary care, mental health support, care for substance use disorders and an on-site pharmacy. Old Town Clinic is adjoined to theOld Town Recovery Center and the Richard Harris Building (8x8) . The clinic opened in 1986 and became a part of CCC in 2001

Schedule: Monday - Friday, 8:00am - 5:00pm (Saturday & Sunday Off)

Location: Old Town Clinic, 727 W Burnside St. Portland

Compensation: $24.30/hr - $33.25/hr, Final comp will be determined based on relevant experience, education, credentials/licensure, and internal equity. External candidates: Offer max is $28.20/hr per our Union contract.

MINIMUM QUALIFICATIONS:

• High school diploma or GED required. PreferBachelor’sDegree or higher in health or social.
• Experience working with homeless, low-income, or other vulnerable populations in a supportive roleor in a clinical setting.
• Spanish language fluency preferred.
• Has the competencies necessary to:
  • Communicate effectively orally and in writing;
  • Understand healthcare related terminology;
  • Provide outreach, engagement and supportive services

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Receive, document and direct patient telephone calls in a complete and timely manner according to clinic protocols.
• Electronically file and index clinical information into the patient’s chart in EMR.
• Obtains, processes, and documents all necessary information and paperwork required toassist theCare TeamManagers (CTM)and providersin the provision of health services to Old Town Clinic patients.
• Provide follow up contact in person or by phone to patients identified byCTM or other care team members for support and management of health related issues.
• Participate in Outreach activities necessary to offer services and engage participation in a non‑coercive manner. This includes outreach to patients who are due for routine preventive services, patients needing follow up for chronic disease management, and patients who have no‑showed for appointments.
• Provide educational information and materials to patients regarding available services in the community and how to appropriately access them.
• Coordinate services with all members of the care team and the client’s support network.
• Maintain client records as required by program standards and funding requirements.
• Participate in training opportunities to advance skills and knowledge inprimary care,addictions and mental health.
• Works within scope of training and experience and seeks consultation when appropriate.
• Attend all mandatory CCC trainings in a timely manner.
• Adhere to all state and federal privacy and security regulations applicable to the program, and to CCC policies and agreements regarding confidentiality, privacy, and security. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
• Perform other duties as assignedordelegated by Care Team Manager.

• Knowledge of de‑escalation methods or ability to be trained in de‑escalation methods.
