What You'll Do:

• Conduct assessments and develop personalized treatment plans for home health patients
• Provide physical therapy modalities and interventions to aid patient recovery
• Educate patients and their families on ongoing care and preventative strategies
• Maintain accurate documentation of patient progress and treatment updates
• Collaborate with a multidisciplinary team to ensure optimal patient outcomes
• Adhere to all safety and professional standards in a home environment

What You Need:

• Valid Texas state license as a Physical Therapist
• BLS certification (or willing to obtain upon hire)
• 1+ year of experience preferred, especially in home health; all applicants considered
• Strong communication skills, compassion, and adaptability
• Ability to work independently in a home setting
