Responsibilities

Provide comprehensive long-term care pharmacy services across assigned facilities.
Review and verify medication orders for accuracy, appropriateness, and regulatory compliance.
Conduct medication therapy management and drug regimen reviews for LTC residents.
Collaborate with nursing staff, physicians, and healthcare providers to optimize resident medication regimens.s
Ensure full compliance with state and federal pharmacy regulations and LTC standards.
Support the daily workflow and operations of the pharmacy system

Experience

Long-term care pharmacy experience preferred
Retail or community pharmacy experience also considered for the right candidate.
Comfortable working in a close-knit community pharmacy environment

Skills

Strong attention to detail and clinical judgment in an LTC setting
Effective communicator with nursing staff, physicians, and long-term care facility teams
Flexible and adaptable across multiple scheduling arrangements
Genuine commitment to the long-term care patient population

Qualifications

Active Minnesota Pharmacist License required or willing to obtain
