Summary:Performs a combination of administrative and data management duties to support grant-related programs. Provides day-to-day administrative and care management support by maintaining organized records, assisting with follow-up activities, and facilitating communication related to patient care within the pediatric primary care environment. Provides coverage and overall support to the care coordination team and program leadership, assisting with caseload management, monitoring referral status, and helping to ensure timely completion of tasks and program requirements. Supports Patient Centered Medical Home efforts including ongoing evaluation of data collection processes and recommends improvements to enhance efficiency, workflow, and reporting accuracy within the grant-funded programs and clinic setting. Supports accurate and timely data entry, maintenance, and tracking of information within databases and reporting systems to ensure compliance with grant requirements.

Education:

Essential:* Associate Degree

Experience:Essential:* Two years related experience
