(1510) Adult Intensive Case Manager - TCOOMMI

Crisis Hotline: View phone number on click.appcast.io | Appointments or Services: View phone number on click.appcast.io The Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) Adult Intensive Case Manager (ICM) is responsible for providing outstanding person‑centered intensive case management services to adults who have a diagnosed severe and persistent mental health disorder, are currently on parole and/or probation, and reside in Caldwell and Gonzales Counties, Texas. The position reports directly to the TCOOMMI Program Director and requires the development and maintenance of effective, professional working relationships with local parole and probation officers and other community service providers. This is a Monday through Friday, non‑exempt, field‑based position with flexible hours – no night shift or on‑call. The TCOOMMI ICM regularly meets individuals in their homes, community, probation/parole, court, and at Bluebonnet Trails’ clinics to provide case management and psychosocial rehabilitative services for a caseload of up to 25 individuals. Services are intended to stabilize mental health symptoms, reduce risk of recidivism, increase the individual’s awareness of and participation with community and natural supports, develop self‑advocacy, and assist with their ability to participate in independent behavioral health services. 55% of hours worked are expected to include billable and non‑billable service hours on a monthly basis to achieve 100% productivity percentages throughout the month. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. The person in this position must possess a passion and aptitude for working with individuals who have chronic and severe mental health needs and are involved in the criminal justice system. Knowledge of psychotherapeutic medications, intensive case management functions, substance use treatment, crisis services, community resources, and evidence‑based practices in the mental health and criminal justice systems are required. This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person‑centered recovery planning and dual‑diagnosis treatment. The person in this position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, balance self‑care, be a delight to work with, and perform other duties as assigned with a positive attitude. Location: Gonzales and Caldwell Counties. Hourly Salary Range: $21.46 – $28.71. Up to $1.00/hour – Previous experience in a similar role Up to $4.00/hour – Tenure with BTCS

Minimum Qualifications A bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education/intervention. Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment. If a degree is not in a Human Services field, a minimum of 24 college credits in a Human Services field are required. NCIC and TCIC criminal background checks, substance use screening, and fingerprinting are performed prior to employment. An official copy of the college transcript is required. Preferred Qualifications Previous experience working in TCOOMMI, behavioral health, criminal justice, and/or case management. Advanced degree in Human Services field. QMHP certification.
