Team members work collaboratively to uphold dignity, respect, and comfort for every client. Role Description The Community Liaison is a full-time, on-site role based in the San Francisco Bay Area. This role builds and maintains relationships with community organizations, healthcare providers, and families to increase awareness of Care Must’s home health and hospice services. Day-to-day responsibilities include conducting outreach visits, attending local events, presenting information about services, and responding to inquiries from potential clients and referral sources. The Community Liaison collaborates with clinical and administrative teams to ensure smooth referrals, supports clients and families in understanding available care options, and gathers feedback to improve community engagement strategies. The role also involves tracking outreach activities, preparing reports, and representing the organization in a professional, compassionate manner. Qualifications Strong community engagement and community outreach skills to build partnerships and increase awareness of services. Excellent communication and customer service abilities to interact effectively with clients, families, and referral sources. Understanding of home care and hospice services, with the ability to explain care options clearly and sensitively. Experience working with diverse communities, ideally within healthcare, social services, or nonprofit environments. Ability to work on-site across the San Francisco Bay Area, including travel to community events and partner locations. Organizational and time-management skills to handle multiple outreach activities and maintain accurate records. Proficiency with basic office software and CRM or outreach tracking tools. High school diploma or equivalent required; post-secondary education in healthcare, social work, communications, or related field is preferred. #J-18808-Ljbffr
