Nurse Practitioner

Role Advanced Practice Clinician within the Senior Community Care (SCC) program, providing care to high-risk health plan members in various settings such as nursing homes and assisted living. Responsibilities Conduct comprehensive assessments, including medical history, medication reconciliation, vital signs, and physical exams. Support members during short-term rehabilitation episodes. Ensure accurate documentation, including ICD-10 coding. Collaborate with the interdisciplinary care team. Lead advanced illness and care planning discussions. Provide counseling and education to patients and caregivers. Qualifications & Skills Active certification and licensure (ANCC, AANP, NCCPA, or state-specific). Unrestricted license and DEA registration or ability to obtain. Reliable transportation and a valid driver’s license. Ability to navigate diverse environments and perform job duties effectively. Preferred: 1+ years clinical experience, especially in geriatrics, long-term care, or home care, with proficiency in electronic medical records.
