Registered Nurse Case Manager (RN CM)

Job Overview

We are seeking an experienced Registered Nurse Case Manager (RN CM) to join our team. The ideal candidate will have a strong background in case management and be capable of handling a heavy case load with minimal supervision.

Shift and Schedule

  • Day Shift: 8:00 AM - 4:30 PM
  • Days: Sunday through Thursday
  • Weekly Hours: 40 hours
Qualifications
  • Education: Bachelor's Degree in Nursing (BSN) preferred.
  • Experience: Previous experience in a care management role is preferred.
  • Licenses/Certifications:

    • Registered Nurse (RN) licensure in the state of practice is required.
    • Case management certification is preferred.
    • Basic Life Support (BLS) certification from an approved vendor per policy is required.
  • Experience with Interventional Program (IP CM): Candidates with experience in IP CM are strongly encouraged to apply.
  • Additional Experience: Preferred candidates will have familiarity with strike force operations for comparable organizations, requiring minimal training.
Key Responsibilities

The Registered Nurse Case Manager will be responsible for managing patient care effectively, ensuring optimal outcomes through coordinated efforts across healthcare settings. Responsibilities include:
  • Overseeing a heavy case load of patients and collaborating with interdisciplinary teams.
  • Conducting assessments and developing individualized care plans.
  • Facilitating communication between patients, families, and healthcare providers.
  • Monitoring patient progress and adjusting care plans as needed.
