LICENSES & CERTIFICATIONS

Medical License:

Arizona licensure requirements to be confirmed
Board Certification:

Board Certified in Family Medicine or Internal Medicine
Board Eligibility:

Truly Board Eligible Family Medicine or Internal Medicne physicians considered
CLINICAL REQUIREMENTS

Provide outpatient primary care to patients ages 0-65.
Must be comfortable treating patients of all ages.
Must be comfortable performing annual women's wellness exams.
Commercial insurance patient population only.
Ramp up to approximately 17-21 patients per day.
Experience with eClinicalWorks preferred.
