Clinical Coordinator RNMake a difference in the lives of patients with chronic kidney disease.The Clinical Coordinator RN will work on-site at DaVita's 6-chair Skilled Nursing Facility dialysis dens. The modality eliminates the need for patient travel to an outpatient dialysis facility. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week, placing a burden on patients and adding avoidable costs to the health care system.DaVita is seeking a Clinical Coordinator RN to lead dialysis care in a skilled nursing facility.
