An organization is seeking a Nurse Practitioner for a Transitional Care Management role in White Plains, New York. This locum opportunity focuses on post-discharge follow-up, care coordination, and supporting safe transitions from hospital to home.

About the Opportunity:

• Shift: Day shift
• Schedule: Monday through Friday
• Hours: Standard business hours
• Setting: Inpatient

Responsibilities:

• Conduct post-discharge follow-up calls with adult patients.
• Assess patient recovery and identify potential complications or concerns.
• Review and reconcile medications as appropriate.
• Reinforce discharge instructions and provide education to patients and caregivers.
• Coordinate care with physicians, social workers, case managers, and other members of the interdisciplinary team.

Qualifications:

• Active Nurse Practitioner license.
• Experience independently managing adult patients.
• Strong clinical assessment, critical thinking, and communication skills.
• Comfortable conducting patient care primarily via telephone while collaborating with an interdisciplinary team.

Desired Skills:

• Background in hospital medicine, primary care, transitional care management, care coordination, internal medicine, or family medicine.

Board Certified or Board Eligible Medical Doctor with or w/o US residency with current US work visa.
