Nurse Practitioner Opportunity
Elite is seeking compassionate and dedicated nurse practitioners to join our team in providing exceptional home health and hospice care. These principles guide our commitment to delivering high-quality, personalized care that allows patients to remain in the comfort of their own homes. As an NP with Elite, you'll play a vital role in "Keeping Our Community Home", fostering meaningful connections, and ensuring patients receive the best possible care. What You'll Do:
- Provide palliative care through evaluation and assessment of health history, diagnosing medical conditions.
- Develop individualized plans of care.
- Complete face to face assessments and prescribe medications or treatments.
- Collaborate with the interdisciplinary team.
What You Bring:
- Active NP certification (state-licensed).
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong communication skills and a team-oriented mindset
- Ability to work independently in home settings
- Commitment to patient-focused care
- CPR Certification
