Nurse Practitioner Opportunity

What You'll Do:

• Provide palliative care through evaluation and assessment of health history, diagnosing medical conditions.
• Develop individualized plans of care.
• Complete face to face assessments and prescribe medications or treatments.
• Collaborate with the interdisciplinary team.

What You Bring:

• Active NP certification (state-licensed).
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication skills and a team-oriented mindset
• Ability to work independently in home settings
• Commitment to patient-focused care
• CPR Certification
