Continuum Care Hospice
Continuum Hospice is seeking a self-motivated Hospice RN Clinical Liaison to join our team in the Boston and South Shore area. You will focus on hospital and medical facility outreach to identify referrals and educate families about hospice services, using clinical knowledge and empathetic communication to support referral sources and patients. You will work to build the hospice brand as the preferred provider, maintain relationships with physicians, clinics, case managers, and social workers, #J-18808-Ljbffr Continuum Care Hospice
