RN - Homecare (Hybrid)

WCS is seeking a Registered Nurse to join a reputable Home Health organization performing Uniform Assessment System (UAS) assessments for a leading Medicaid assessment organization. This is a Full Time Monday through Friday 9am-5pm role, working 3 days a week in the field and 2 days a week from home.

This role involves conducting in-home field assessments with a hybrid schedule, offering three days on-site and two days remote each week. The position provides an excellent opportunity to work with one of New York's most respected organizations while maintaining work-life balance.




Job Function:

• Schedule: Hybrid | 3 days on-site 2 days remote | Monday to Friday | 5d x 8hrs flexible hours (8:30am-4:30pm, 9:00am-5:00pm, or 10:00am-6:00pm)
• Conduct preliminary in-home evaluations for consumers.
• R eview assessments to determine if the correct conclusion for eligibility of services was reached by the nurse assessor.
• Collect data related to assessments and assessment outcomes.
• Examine each case for completeness, accuracy and consistency.
• Analyze UAS recommended scores in relation to nurse assessor determinations.
• Conduct scheduled home visits with the nurse assessor.
• Examines data for outliers and researches such cases.
• Develop/complete reports that track and trend activities/outcomes.

• Required Education NYS RN Licensure
• 2 years of RN homecare experience required
• Must be able to travel to Queens.
