Job Description
Title
LPN/LVN - Travel

Schedule
Shift: 5x8, 1500 (3:00 PM) - 0330 (3:30 AM)
Weekends: Every other weekend
Holiday: Available to work every other holiday, if needed

Assignment Details
Contract Length: 13 weeks

Requirements
- Active New York LPN/LVN license (single-state; no pending)
- BLS certification
- Minimum 1 year nursing experience
- Previous charge experience
- Travel experience
- LTAC experience
- EPIC charting experience required

Roles & Responsibilities
- Provide skilled nursing care to patients in a Long-Term Acute Care environment.
- Administer medications via IM and SQ routes.
- Monitor vital signs and perform blood glucose testing.
- Assist patients with activities of daily living (ADLs) and ambulation.
- Care for feeding tubes and perform Foley catheter insertions.
- Report changes in patient status to supervising RN or MD.

Compliance / Notes
- Orientation: 4 hours
- Must disclose RTO requests at the time of submission
- Background checks: reviewed on a case-by-case basis
- Scrub color: Navy blue and white combinations acceptable
