POSITION OVERVIEW You'll travel to skilled nursing facilities, rehabilitation centers, assisted living communities, memory care facilities, and patient homes to provide comprehensive wound assessments and treatment. This position offers the perfect blend of clinical autonomy, schedule flexibility, and meaningful patient care—allowing you to practice at the top of your license while collaborating with our Medical Director and clinical leadership team. WHAT YOU'LL DO Clinical Care Perform comprehensive assessments for acute and chronic wounds, including: Pressure injuries, diabetic ulcers, venous and arterial ulcers Surgical, traumatic, and infection-related wounds Develop and implement evidence‑based treatment plans. Perform wound care procedures including: Selective and non‑selective debridement Dressing applications, compression therapy Negative Pressure Wound Therapy (NPWT/VAC) Advanced wound care treatments Monitor healing progress and adjust treatment plans as needed. Patient & Caregiver Education Educate patients, families, caregivers, and facility staff on wound management and prevention. Promote treatment plan adherence through ongoing communication and support. Collaborate with physicians, home health agencies, facility staff, hospice teams, and interdisciplinary care teams. Participate in monthly clinical meetings and case reviews. Documentation & Compliance Complete timely and accurate documentation within the EMR. Maintain compliance with Medicare regulations, HIPAA, infection control standards, and state practice requirements. Ensure proper coding and documentation of services. QUALIFICATIONS Required Active, unrestricted Nurse Practitioner license in South Dakota (multi‑state license strongly preferred) National Board Certification (FNP, AGNP, or equivalent) Minimum 1 year of Nurse Practitioner experience Reliable transportation, valid driver’s license, and current auto insurance Strong clinical assessment and critical‑thinking skills Ability to work independently in a mobile healthcare environment Excellent communication and documentation skills Preferred Wound Care Certification Experience in wound care, home health, long‑term care, hospice, mobile healthcare, or post‑acute care settings Familiarity with Medicare guidelines and wound care documentation requirements WORKING ENVIRONMENT This is a mobile healthcare position serving patients in: Skilled Nursing Facilities Assisted Living Communities Memory Care Facilities Providers should be comfortable traveling throughout assigned territories and working independently while maintaining high standards of clinical excellence. COMPENSATION Equivalent earning potential of approximately $50-60+/hour based on productivity. No cap on earnings - as our patient census grows, so does your income.
