Job Opportunity: Licensed Practical Nurse (LPN)

Our organization is in search of a dedicated and empathetic Licensed Practical Nurse (LPN) to become a vital part of our Healthcare & Medical Services team. This position involves delivering high-quality, resident-focused nursing care to individuals in a long-term care and post-acute setting, aligning with our mission to transform healthcare through innovation, ongoing improvement, and cultural enrichment.

Key Responsibilities

• Provide hands-on clinical care, including monitoring vital signs and administering prescribed medications and treatments.
• Perform wound care and assist residents with activities of daily living (ADLs).
• Promptly report any changes in residents' conditions to the supervising Registered Nurse and healthcare provider teams.
• Maintain accurate documentation of care and assessments in electronic medical records, ensuring adherence to regulatory standards and facility policies.
• Collaborate closely with Registered Nurses, physicians, therapists, Certified Nursing Assistants (CNAs), and other interdisciplinary team members to create, implement, and update personalized care plans that reflect each resident’s goals and clinical needs.
• Educate residents and their families about disease processes, medications, and care routines, fostering dignity, independence, and an enhanced quality of life.

• Ability to prioritize care effectively in a dynamic, on-site environment.
