Licensed Practical Nurse Intake Coordinator

Sentara Home Care is seeking an LPN Intake Coordinator with flexible weekend availability, located in Chesapeake, VA. The role involves evaluating referral calls, recording outcomes, updating referral data, and serving as a resource for patients and referral sources. Job Details Location: Remote with occasional onsite meetings and trainings in Chesapeake, VA. Eligible candidates must live within one hour of Chesapeake, VA. Hours: Weekend days, flexible as needed. Compensation: $17.31 - $28.80 per hour plus incentives and benefits. Responsibilities Receive and process incoming referral calls from sources or prospective patients. Record call outcomes and perform follow-up calls as necessary. Enter referral information, including clinical data and service orders, into the system. Maintain up-to-date knowledge of all services and resources offered by Sentara and the community. Act as a resource for addressing patient and referral source issues. Qualifications High school graduate or equivalent. LPN license, active in Virginia. Minimum 1 year of clinical and customer service experience in a healthcare environment. Strong care coordination experience preferred. Home health experience preferred.
