Job Description Position: Physical Therapist Type: Direct hire – permanent, or a 13‑week traveler assignment (either acceptable). Employment: Full time Compensation: Competitive, based on experience. Responsibilities include ensuring the health and well‑being of residents by providing comprehensive, quality therapy services. A state‑specific Physical Therapy license is required. Responsibilities Initiate skilled physical therapy treatments according to physician's orders; maintain required contact with physician. Maintain written evaluations, progress notes and discharge summaries for residents in the physical therapy program. Confere with other health‑care staff regarding special needs of residents to enhance rehabilitation or maintenance of function. Provide input for Resident Care Planning meetings and attend during absences of the Director of Physical Therapy. Maintain statistical records of resident treatments. Assist with in‑service training programs upon request. Record and update residents' plan of care in the Inter‑disciplinary Kardex. Submit requisition for supplies needed in the treatment of residents to the Director of Physical Therapy. Assume all duties of the Director of Physical Therapy during absences, including supervision of Physical Therapy Aides and responsibility for all necessary documentation. Qualifications Graduation from an approved school of physical therapy required. State license to practice physical therapy required. Two years’ experience in physical therapy preferred. Interest in physical therapy treatment programs for the geriatric and subacute populations necessary. Ability to direct others necessary. #J-18808-Ljbffr
