Job Description

We are seeking a qualified Home Health Physical Therapist to join our team. The ideal candidate will have experience in home health settings and familiarity with OASIS documentation. This role involves visiting patients in their homes to provide therapy and rehabilitation services, ensuring they receive the highest level of care.

Responsibilities

• Conduct evaluations and create individualized treatment plans for patients.
• Implement therapy interventions to improve patients' mobility and quality of life.
• Document patient progress and update treatment plans as required.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Manage a caseload of 4-6 visits per day, ensuring productivity targets are met.

Required Experience / Certifications / Licensure

• Home Health and OASIS experience required.
• Homecare Homebase documentation experience preferred.
• Licensed Physical Therapist in California.
