Responsibilities
- Conduct 1-hour patient evaluations and 45-minute daily treatment sessions
- Develop and implement individualized plans of care for orthopedic, sports, and work-related injuries
- Collaborate with a multidisciplinary rehabilitation team to coordinate patient care
- Document care in EPIC; EPIC experience preferred
- Manage referrals from orthopedic surgeons, family practice providers, and the acute care team
- Monitor patient progress and adapt treatment plans to achieve optimal outcomes

Requirements
- Doctor of Physical Therapy (DPT) or PT degree from an accredited program
- PT licensure: active license or eligibility to obtain licensure as a Physical Therapist per state regulatory requirements
- 1+ year of physical therapy experience; outpatient experience preferred
- Ability to work effectively in an outpatient hospital-based rehabilitation setting
- Strong communication and teamwork skills
- EPIC experience preferred
