Acute Care Case Manager

This is a 13-week contract position.

Day shift, 5 days a week from 08:00 AM to 04:30 PM, with weekend coverage required every other weekend.

The Acute Care Case Manager plays a crucial role in the discharge planning process and must effectively manage a caseload of up to 25 patients, ensuring timely and coordinated patient care. The position involves collaboration with multi-disciplinary teams and coordination of care across various services to facilitate smooth transitions of care for patients.

• Conduct biopsychosocial assessments and develop individualized treatment plans.
• Facilitate discharge planning and contingency planning.
• Coordinate services to home care and equipment following discharge.
• Document care delivery, plans, and patient interactions in the electronic charting system.
• Educate patients and caregivers about care processes and resources available.
• Participate in family meetings and collaborate with external resources as needed.

• 2 years of acute hospital case management experience required.
• Experience with the Epic charting system.
• Knowledge in handling involuntary holds and psychiatric assessment protocols.
• CA state license preferred; pending license accepted.
