RN Care Manager

RN Care Manager (Registered Nurse) Fairhope, Alabama Altera Health & Rehabilitation Center is seeking a compassionate and experienced RN Care Manager to join our clinical leadership team in Fairhope, Alabama. In this role, you will coordinate resident care from admission through discharge, collaborate with the interdisciplinary team, and support quality outcomes for our short-term rehabilitation and long-term care residents. What You'll Do Coordinate resident care from admission through discharge Collaborate with physicians, nursing, therapy, admissions, and social services Lead interdisciplinary care plan meetings Assist with discharge planning and transitions of care Coordinate care for Medicare, managed care, and long-term care residents Communicate with residents and families about care plans and progress Maintain accurate, timely clinical documentation and regulatory compliance Support quality improvement initiatives and help reduce avoidable hospital readmissions Qualifications Current Alabama Registered Nurse (RN) license required Experience in skilled nursing, rehabilitation, long-term care, case management, MDS, admissions, or care coordination preferred Strong clinical assessment, communication, and organizational skills Ability to manage multiple priorities and work collaboratively with an interdisciplinary team Why Join Altera?
