Roles & Responsibilities

Provide top-notch physical therapy services to residents in a skilled nursing facility
Develop and implement individualized treatment plans to improve mobility and function
Collaborate with interdisciplinary teams to ensure comprehensive patient care
Document patient progress and adjust treatments for optimal outcomes
Educate patients and families on therapeutic exercises and home programs

Qualifications

Active Physical Therapist (PT) license in Minnesota or compact eligibility
Doctor of Physical Therapy (DPT) degree preferred
New graduates welcome to apply - we support your growth!
Previous SNF experience is a plus but not required

Knowledge & Skills

Expertise in patient assessment and treatment planning
Strong communication and teamwork abilities
Knowledge of rehabilitation techniques for geriatric populations
Proficiency in electronic medical records (EMR) systems
