PDN / Private Duty Nurse (LPN or RN) - Home Health, 8hrs/wk or Part-time

JOB TITLE: Home Health Visit Nurse (RN or LPN) Required Qualifications Active Ohio LPN or RN in good standing. At least 1 year of paid experience at a home health agency (or equivalent); 2‑5 years preferred. Experience in Case Management, wound care, PICC/PORT/IV Care, lab draws, OASIS and Non‑OASIS assessments; Nurse supervision experience is a plus. Minimum 1 year direct patient experience with ventilator care. Good interpersonal communication skills – written and verbal. Active CPR and First Aid certificates (highly desired; agency will provide recertification). Current driver’s license or state ID and reliable transportation. Excellent attendance record with past employers. Ability to pass state and federal background check. Willingness to meet all basic job requirements. Schedule & Travel Requirements Typical schedule: Monday‑Friday (weekdays) from 8 am to 5/6 pm. Part‑time PDNs: minimum travel radius within Montgomery County and surrounding areas 30‑40 minutes (up to 60 minutes on occasion). Full‑time PDNs: minimum travel radius up to a 2‑hour radius (additional cases may be added if desired). Qualified applicants may reside in or near any primary service area as long as travel requirements are met. Employment Type Long‑term employment for full‑time or part‑time positions. No on‑call requirements / no overnight shifts. No required weekends (optional when available). Overtime, respite & PRN services may be offered but are not required. Position Summary A successful PDN at Home Care Network, Inc., performs skilled nursing functions in accordance with federal, state, and local laws and agency policies. The LPN/RN delivers care to clients under the direction of the RN Case Manager. Reports To RN Case Manager; Clinical Administrator/Director of Nursing Essential Functions Provides professional nursing care under the direction of a Registered Nurse and communicates status changes. Performs skilled nursing visits and follows the care plan established by the RN. Recognizes and reports changes in client condition to the Case Manager and physician. Communicates timely information per agency policy and urgency. Scopes include personal care, assessment, medication monitoring & administration, client teaching, emergency intervention, and nursing services within professional scope. Individuals with “LPN M‑IV” designation may perform venipuncture for lab samples. Promotes personal safety and a safe environment for clients and coworkers. Practices safety/infection control and responds appropriately to unsafe situations. Demonstrates safe use of equipment and recommends safety improvements. Complies with agency policies and professional standards. Accepts professional nursing responsibility. Follows applicable state and federal laws. Attends required meetings. Maintains confidentiality. Refrains from discussing client or staff identities publicly. Participates in the Agency Quality Improvement Program. Performs other related duties as necessary. Essential Physical Demands Frequent standing, walking/bending/stooping, occasional sitting.
