WELLNESS COACH II

Continuous Recruitment Job Summary Under the direction of an assigned administrator and in coordination with a Pupil Personnel Services credentialed professional, operate as part of a care team to provide support to students in a Pre‑K‑12 environment in the areas of behavioral health and well‑being, offer prevention and early intervention services that support the behavioral health and well‑being of children and youth in the areas of wellness promotion and education, screening, care coordination, individual and group support, crisis referral, supporting youth in skills training, repetition, and practicing skills that are learned in higher‑level care. Essential Duties & Responsibilities Design and facilitate targeted, developmentally appropriate lessons using evidence‑based practices that foster emotional resilience, self‑regulation, and peer respect in alignment with Multi‑Tiered Systems of Support (MTSS). Utilize trauma‑informed approaches during student interactions to identify emerging needs, track progress toward individual goals, and document interventions using district approved platforms. Establish and maintain cross‑sector partnerships to enhance service delivery, identify gaps, and align resources with strategic wellness goals and community initiatives. Coordinate multidisciplinary communication among educators, counselors, mental health professionals and external providers to support continuity of care and comprehensive student wellness planning. Serve as a liaison to bridge communication among stakeholders, ensuring student needs are addressed holistically, through collaborative planning and culturally responsive practices. Actively contribute to Student Study Team (SST) or Individualized Education Plan (IEP) meetings to provide insight on wellness supports and reinforce shared accountability. Track student progress in PBIS programs using data collection tools and collaborate with staff to adjust intervention strategies based on outcome trends. Conduct initial triage and provide stabilization support during mental health crisis, ensuring alignment with school policies and legal reporting mandates. Conduct targeted outreach through home visits to address attendance, wellness, or academic concerns, fostering family partnerships and trust. Implement brief, goal‑focused sessions using motivational interviewing and psychoeducation techniques to support early intervention and staff self‑efficacy. Deliver small group wellness programming focused on developing social‑emotional skills, stress management, and life skills through structured interventions such as behavioral activation, mindfulness, and emotional regulation techniques; support students in identifying and engaging in activities that promote well‑being and practicing adaptive coping strategies. Coordinate with school personnel and collaborate on behavioral screenings; identify and refer students for behavioral health services within the school or broader organization. Process documents such as intake forms and billing for activity documentation, information dissemination, and program compliance. Collaborate with district, community organizations, and other entities for planning, implementing, and maintaining program services. Intervene in crisis situations following established procedures to ensure safety and reduce crisis intensity while maintaining confidentiality. Present information to district and community groups to promote programs, gain feedback, and meet internal requirements. Coordinate care, connecting individuals to internal and external behavioral health resources, and facilitate communication with professionals. Attend meetings with students, parents, and staff as needed. Support parent and community engagement initiatives. Communicate with parents and staff as appropriate within confidentiality limits. Document interventions and bill for services as directed. Meet with students individually (less than 30 minutes) for wellness promotion, mental health literacy, and life skills education. Other Duties Perform related duties as assigned for the purpose of ensuring the efficient and effective functioning of the work unit. Required Qualifications Wellness Coach Certification or ability to become certified through the California Department of Health Care Access and Information (HCAI). Bachelor’s Degree in Social Work, Human Services, Addiction Studies, Psychology, or any field. 800 verifiable hours of experience within the last six years providing behavioral health services to children and youth through age 25 in a school‑linked or school‑based organization, non‑profit, communication organization, or health center; OR 2,700 verifiable hours of experience within the last six years providing such services. Some positions may require the ability to speak, read, and write in a designated second language. Knowledge Of Comprehensive wellness frameworks, including the interconnection between physical, emotional, and social health and their influence on academic performance; emphasis on applied strategies for stress management, emotional regulation, and executive functioning in school‑aged populations. Child and adolescent mental health disorders, diagnostic indicators, trauma‑informed approaches, and strength‑based interventions that promote resilience, coping skills, and long‑term well‑being in diverse student populations. Behavioral health assessment processes, including administration and interpretation of universal and targeted screenings, and procedures for effective referrals, follow‑up, and continuity of care. Developmental psychology focused on the cognitive, emotional, and social growth stages of children and adolescents and how they inform appropriate wellness interventions, decision‑making support, and problem‑solving skills development. Crisis response and stabilization protocols, including risk assessment, safety planning, and application of culturally responsive, trauma‑informed de‑escalation techniques. Program compliance and documentation practices, including accurate completion of intake forms, service logs, billing procedures, and data entry within electronic health record (EHR) systems, in accordance with district, funding, and legal requirements. School‑wide behavioral support systems, including advanced understanding of Positive Behavioral Interventions and Supports (PBIS) and Tier 2/3 interventions such as Check In/Check Out and Renew, along with data collection for monitoring intervention efficacy. Professional ethical standards and confidentiality laws for working with minors, including FERPA, HIPAA, and district‑specific guidelines for safeguarding student information and maintaining trust in school‑based mental health services. Certified Wellness Coach Code of Ethics and district policy, including state and federal regulations governing school wellness services, ethical decision‑making models, and practices that actively promote diversity, equity, inclusion, and social justice within educational environments. Mental health conditions, wellness strategies, and resilience building in children and adolescents. Anti‑bullying programs, strategies, causes, impacts, and prevention methods in educational settings. Child and adolescent development, particularly in emotional and social skills, problem‑solving, and decision‑making. Crisis management techniques and protocols, including de‑escalation strategies and maintaining a safe environment. Positive Behavioral Interventions and Supports (PBIS), and support for interventions like Check In/Check Out and Renew. Ability To Relate to and work with at‑promise youth, advocate on behalf of the target population. Provide individual and group support for all students. Coordinate various student services with appropriate agencies. Prepare and maintain a variety of records and reports, including confidential materials. Adhere to District policies, procedures, and Certified Wellness Coach Code of Ethics. Read, interpret, apply, and explain laws, rules, regulations, policies, and procedures. Analyze situations accurately and adopt an effective course of action. Work independently with little direction, work as part of a team, and work with frequent interruptions. Meet schedules and timelines. Work confidentially with discretion. Develop and maintain cooperative, constructive relationships with students, parents, staff, and outside agencies. Effectively communicate orally and in writing with diverse groups of students, parents, staff. Operate a computer and other office equipment.
