Responsibilities:

• Conduct initial patient assessments, develop personalized care plans, and provide skilled nursing care to patients in their homes

• Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to ensure comprehensive care

• Administer medications, perform wound care, and provide education to patients and their caregivers

• Monitor patients' progress, report changes, and make recommendations for adjustments to care plans

• Maintain accurate and detailed records of patient interactions, treatments, and progress

Requirements:

• Current registration with the Illinois Board of Nursing (IBON) as a Registered Nurse (RN)

• Valid Driver's License (DL) and reliable transportation

• Minimum 0 years of experience in home health or a related field (new graduates welcome to apply!)
