Schedule: - Shift: 5:00 PM - 8:30 AM - Flexible Monday - Friday, 8:30 AM - 5:00 PM - Will consider 4/10s

Assignment Details: - Contract Length: 13 weeks - Guaranteed Hours: Not specified

Requirements: - 1+ year experience in hospice - Valid driver's license - Good driving record and reliable transportation

Roles & Responsibilities: - Appropriately handle calls either by phone or with home or facility visits when necessary or requested. - Provide education and emotional support for patients and families. - Electronically communicate all activity upon completion of on-call shifts. - Document pertinent calls and visits electronically.
