Registered Nurse - Hospice

Job Summary:
The Hospice Registered Nurse provides compassionate in-home care and emotional support to patients experiencing life-limiting illnesses. The nurse is responsible for coordinating patient care, educating families, and ensuring compliance with regulatory requirements.

Responsibilities:

Assess patient needs and develop individualized care plans.
Deliver direct nursing care in patients' homes or facilities.
Educate patients and families on disease management and symptom control.
Document all clinical activities and patient interactions electronically.
Communicate effectively with on-call team members and healthcare providers.
Support patients emotionally and provide respite for families as needed.

Qualifications:

Hold a valid Virginia RN license.
Possess a minimum of 1 year of hospice experience.
Maintain a valid driver's license with a good driving record.
Demonstrate strong interpersonal and communication skills.
Ability to work independently and manage multiple patient cases.
