Come join our growing home care agency in Mason, OH. Beulah Home Care LLC is seeking an outgoing, ambitious, motivated and forward-thinking individual to join our team as a Service Coordinator. Monday through Friday 8:30 a.m. to 4:30 p.m.
Core Responsibilities
Oversee the intake process for all new clients, ensuring timely and accurate eligibility verification and service set up.
Receive and review referrals for new clients, verifying eligibility for waiver services and confirming active service authorizations with the relevant county.
Conduct intake assessments to gather comprehensive information about the client’s care needs, medical history preferences and support network.
Coordinate with the family, county coordinator and medical providers to complete all required intake documentation within the timeframe specified by DODD, Medicaid, Medicare, Private Insurance.
Ensure all intake documentation is filed accurately in the client's record prior to the first service delivery date.
Attend county-level team meetings or care conferences for assigned clients as required.
Serve as the primary point of escalation for any service delivery issues, disputes or authorization delays affecting clients.
Follow all Standard Operating Procedures pertaining to Beulah Home Care LLC and comply with all applicable federal and Ohio state regulations.
Previous experience in Home Care/ Health Care, Program Management, Case Management, Medicaid and waivers is a Plus!
