Oak Street Health seeks a Behavioral Health Specialist to provide integrated mental health services for adult Medicare patients in underserved communities. In this bilingual English-Spanish role, you will conduct psychosocial assessments, deliver individual and group therapy, and create evidence-based treatment plans. Collaborating closely with primary care providers, nurses, and care teams, you’ll support patients with chronic conditions, social stressors, and behavioral health needs.
