Position: RN Home Health

We are seeking a dedicated Registered Nurse (RN) to provide high-quality home health care services. The RN will coordinate and direct nursing care according to physicians' orders and agency policies and procedures, utilizing the nursing process competently.

Key Responsibilities:

• Coordinate and direct the provision of home nursing care.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Perform home visits and deliver patient care in compliance with agency standards.
• Complete all required documentation within 24 hours of each patient appointment.
• Ensure patient care aligns with physician orders and agency protocols.

Position Details:

• Shift: Day 5x8-Hour (08:00 AM - 05:00 PM)
• Average Number of Patients per Day: 5-10
• Travel Requirements: Must travel up to 75 miles from the assigned care center. Mileage reimbursed at the IRS standard rate.
• Location: Cleburne and parts of Randolph Counties, AL
• Contract Length: 12 weeks

Qualifications:

• Minimum of 1 year of adult home health experience or 1 year of hospice experience, depending on the specialty.
• Must hold a valid nursing license and have it in hand.
• Proficiency with HomeCare HomeBase (HCHB) and OASIS is required; recent usage within the last 6 months is preferred.
• CERTIFICATIONS: CPR and BLS are required.
• Open to on-call rotations and weekend shifts as required.

Additional Information:

• This position involves driving to patients' homes daily, as well as attending periodic office meetings.
• Local candidates are welcomed to apply.
• There are no guaranteed hours; candidates must meet productivity expectations.
• Documentation must be completed timely to ensure continuity of care and proper billing.
