Occupational Therapist (OT) - Home Health

Schedule: Monday – Friday | 8:00 AM – 4:30 PM (40 Hours/Week) Weekend & Call Requirements: Weekend rotation as assigned Position Overview A regional home health agency in the Fort Dodge, IA area is seeking a full-time, permanent Occupational Therapist (OT) to deliver personalized home-based therapy services. Working directly in patients' home environments, the OT conducts comprehensive clinical assessments, establishes physician-directed treatment plans, and implements functional therapy to improve daily independence and quality of life. Core Responsibilities Home Visits & Assessment: Conduct home visits to perform ongoing interdisciplinary evaluations and formal functional assessments for home health patients. Plan of Care & Treatment: Formulate and execute personalized occupational therapy programs and functional activities in accordance with written physician orders. Environmental Modifications: Evaluate home environments and administer adaptive modifications to promote mobility and home safety. Assistive Technology & Modalities: Oversee the proper selection, fitting, and use of therapeutic appliances, prosthetic devices, and treatment modalities. Care Coordination & Documentation: Collaborate closely with attending physicians, nursing staff, and interdisciplinary care teams to track patient progress and update treatment plans. Candidate Submission & Qualification Requirements Education & Certifications: Education: Graduate of an ACOTE-accredited Occupational Therapy educational program [REQUIRED]. National Certification: NBCOT entry-level certification exam completion or eligibility [HIGHLY PREFERRED]. Life Support: Active AHA Basic Life Support (BLS) Healthcare Provider certification [REQUIRED]. Mandatory Training: Mandatory Reporter Certification for Child & Dependent Adult Abuse [REQUIRED]. Licensure & Logistics: State Licensure: Active, unencumbered Iowa Occupational Therapy License (or active Compact License privileges) [REQUIRED]. Driving Requirements: Valid driver’s license with active automobile insurance meeting state and organizational standards [REQUIRED]. Clinical Experience: Experience Level: Minimum of one (1) year of post-licensure clinical experience caring for a similar adult/geriatric home health or rehab population [REQUIRED]. #J-18808-Ljbffr
