Contract | Days (8:00 AM – 4:00 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled occupational therapy services to patients in their homes across an assigned coverage area, including Maricopa, Casa Grande, and Eloy, with flexibility to travel to other areas within the East Valley team
• Evaluate patients and develop individualized treatment plans to support recovery from surgery, chronic illness management, or adjustment to new medical treatments
• Deliver direct patient care in accordance with physician orders and home health care standards
• Maintain accurate and timely clinical documentation in compliance with Medicare-certified home health requirements
• Collaborate with interdisciplinary care team members to coordinate patient care and progress toward goals
• Maintain active Arizona OT license with no disciplinary actions on record

Required Qualifications

• Minimum of 24 months of occupational therapy experience, with experience within the last 12 months
• Active Arizona Occupational Therapist license in good standing (no hits on state license), verified within the last 30 days
• Active BLS certification from an approved vendor (AHA, ARC, or ASHI)
• Valid driver's license required
• OT skills checklist completed within the last 12 months
• Arizona residents are not eligible for this assignment

Preferred Experience / Skills

• Prior travel therapy assignment experience highly preferred
• Experience in home health or community-based care settings
• Comfort with flexible scheduling and willingness to cover multiple geographic areas within the East Valley
