Humana's CenterWell Senior Primary Care is seeking a Physician Collaborator for a fully remote role, reporting to our Associate Director. You will collaborate with APPs who provide virtual wraparound services to support patients who have had fractures in the last 6 months when they cannot get in with their own PCP.You will also participate in occasional travel to Humana offices for training or meetings. This is a remote, part-time commitment with an average of 5 hours per month.J-18808-Ljbffr Humana Inc
