Key Responsibilities

Evaluate patients in a home health setting to assess functional abilities and develop individualized treatment plans
Provide therapeutic interventions aimed at improving patients' ability to perform activities of daily living
Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Collaborate with interdisciplinary care teams to ensure coordinated and comprehensive patient care
Educate patients and caregivers on therapeutic techniques and home safety strategies

Required Qualifications

Current and valid Occupational Therapist licensure in the state of Iowa
Graduate of an accredited Occupational Therapy program
Current NBCOT certification
Experience working in a home health or similar clinical setting
Strong documentation and communication skills

Preferred Experience / Skills

Prior home health experience with a diverse patient population
Familiarity with home health electronic medical record systems
Strong ability to work independently and manage a patient caseload in a community-based environment
