Job Responsibilities

• Delivering nursing care in post-acute facilities
• Following resident care plans with consistency and accountability
• Administering medications, verifying dosages, and monitoring their effects on residents
• Monitoring the physical and mental wellbeing of residents, including tracking vital signs
• Participating in IV setup, immunizations, and phlebotomy procedures
• Reviewing and updating residents' charts and medical records as needed
• Conducting resident intakes and accurately documenting health concerns and medical histories
• Helping to foster a safe and comfortable environment within the facility

Qualifications and Skills

• Current and applicable state-issued LPN license
• Completion of an accredited LPN program
• A minimum of 6 months of experience in an LPN or related role
• Ability to lift a minimum of 50 lbs
