Case Manager

Houston, Texas Full-Time Entry-level $40,000–$50,000 Department and Classification Department: Case Management | Reports To: Clinical Director / Program Director / CPMS | FLSA Status: Exempt Position Summary The Case Manager counsels and aids children in care requiring foster placement through Houston Strong by providing comprehensive, T3C-aligned, TBRI-based case management services. The Case Manager is responsible for safety, wellbeing, educational support, permanency planning, service plan implementation, and direct support and coaching for foster families. Essential Duties and Responsibilities Provide case management services to children in care and assist staff with regulatory compliance. Facilitate placement of children in matched, appropriately verified foster homes. Participate in admission assessments, service plans, reviews, discharge planning, and aftercare planning. Maintain regular contact with foster families and children, including at least two monthly home visits. Provide resource information, referrals, crisis intervention, and rotating on-call support. Coordinate medical, psychiatric, educational, court, treatment-team, and service-planning requirements. Complete case notes, incident reports, service plans, education portfolios, medication-log reviews, and other required documentation on time. Maintain compliance with DFPS Minimum Standards, T3C Blueprint requirements, and SSCC contract obligations. Required Qualifications Bachelor's degree in social work, psychology, counseling, sociology, or another human-services field. Related experience working with children or families in child welfare, human services, or a therapeutic setting is preferred. Valid Texas driver's license, appropriate vehicle insurance, and a reliable personal vehicle. Ability to pass required drug tests, background checks, and DFPS background screens. Ability to complete all required pre-service training within applicable timeframes. Skills and Competencies Strong written communication for case notes, service plans, reports, and correspondence. Ability to build trust with children and foster families using TBRI-aligned relationship skills. Ability to interpret professional literature, regulatory documents, assessments, and technical instructions. Demonstrated cultural competency when serving children and families from diverse backgrounds. Physical Demands and Work Environment The role requires regular computer use, standing, walking, reaching, close vision, travel to home visits in varied residential settings, and use of a personal vehicle for work purposes.
