THE GROWTH YOU HELP PROVIDE AS A CLINICAL CARE COORDINATOR

In this full-time role, you'll serve as a trusted partner to members navigating complex healthcare needs. You'll assess individual situations, develop personalized care plans, coordinate resources and services, collaborate with physicians and care teams, and help remove barriers that may impact treatment and recovery. Throughout the day, you'll monitor progress, identify opportunities to improve outcomes, support quality initiatives, and advocate for members to ensure they receive effective, efficient, and coordinated care.

OUR IDEAL CLINICAL CARE COORDINATOR

We're looking for someone who can meet the following qualifications:

  • Bachelor's degree or equivalent combination of education and related experience
  • Licensed Medical Professional
  • Current Certified Case Manager (CCM) certification or able to obtain within 12 months of hire
  • Strong communication, problem-solving, and care coordination skills
  • Ability to manage complex, chronic, and/or catastrophic medical cases
  • Knowledge of medical coding, regulatory guidelines, and evidence-based clinical practices
  • Proficiency with Microsoft Office applications
  • Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents
  • Ability to effectively present information to top management, public groups, and/or boards of directors
  • Strong math skills
