RN Case Manager: Discharge & Care Coordination

St Rose Hospital

St.-Rose-Hospital in Hayward, CA seeks an experienced ER Case Manager to join our Case Management/Social Services team. Under general supervision, you will assist in carrying out the hospital's Case Management Plan, coordinate discharge planning and utilization management, and act as liaison with payers and community resources. The role requires an RN license, BLS, and professional certification in case management is preferred; bilingual English/Spanish is a plus. #J-18808-Ljbffr
