Home Health Physical Therapist

The Physical Therapist uses clinical expertise, physical assessment, teaching, and skilled therapy interventions to deliver high-quality patient care in the patient's place of residence. The Physical Therapist develops or contributes to the development of the home health plan of care under the direction of the physician. The Physical Therapist facilitates appropriate interdisciplinary involvement to ensure the best outcomes for each patient. The Physical Therapist delivers therapy intervention and treatment directed by the physician as established in the home health plan of care. The Physical Therapist treats clients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments and modalities. The Physical Therapist ensures quality and safe delivery of services to patients according to the care plan. The Physical Therapist promotes patient independence by teaching patients and family members about treatment and disease management. The Physical Therapist provides the services that are ordered by the physician as indicated in the plan of care. The Physical Therapist develops and continually updates a plan of care that specifies the care and services required to meet patient-specific needs. The Physical Therapist assesses the need for plan of care updates during every visit and communicates plan revisions to patients and caregivers. The Physical Therapist maintains accurate and timely documentation of clinical records and time entries. The Physical Therapist communicates with all physicians involved in the plan of care and other healthcare practitioners. The Physical Therapist identifies patient risks for hospitalization and proactively intervenes to prevent adverse events. The Physical Therapist supervises home health aide and physical therapy assistant staff and documents supervision in the patient medical record. The Physical Therapist delivers quality care and completes clear, concise, and accurate clinical documentation in accordance with home health conditions of participation. The Physical Therapist participates in clinical review of records to monitor quality and accuracy of care.
