Mandatory Experience:

• Minimum Data Set (MDS) Coordinator works in conjunction with physicians, other nursing staff, residents, families and other facility departments to assure that the resident assessment tool, resident care plan and interdisciplinary team meeting are completed according to all Federal/State regulations and according to facility policy/procedure in order to provide optimum individualized nursing care for the residents of SNF service.
• Preferred: 1 year verifiable experience as a licensed nurse providing care and services in a skilled nursing or long-term care setting. Previous experience in MDS.
• Required: Must possess, as a minimum, a Nursing Degree from an accredited college or university. Must possess a RN/LVN California License. Current CPR.

Preferred:

• MDS certification
• Current RN California License

Knowledge/Skills/Abilities:

• Working the floor and performing daily RN duties.
• Must be a skilled communicator, director, and motivator.
• Able to organize and prioritize many tasks effectively.
