Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate clients' mental health needs, identifying strengths, challenges, and available community resources.

• Facilitate individual, family, and group therapy sessions utilizing evidence-based practices such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care to achieve desired therapeutic outcomes.

• Development and execution of personalized treatment plans, ensuring goals align with clients' specific needs and circumstances.

• Collaborate effectively with a multidisciplinary team, including physicians, nurses, and therapists, to coordinate and enhance patient care.

• Provide crisis intervention and risk assessment, ensuring the safety of clients during critical times.

• Maintain meticulous documentation of all client interactions, assessments, and treatment progress in compliance with ethical and legal standards.

• Lead group therapy sessions, psychoeducational workshops, and support groups to encourage peer support and communal healing.

Patient Population / Client Focus\n

This role centers primarily on patients receiving hospice care, including individual adults and families navigating the complexities of terminal illness. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in New York.

Compensation and Benefits\n

This position offers a competitive salary range of USD 80,000 – 100,000 annually, along with a signing bonus to attract top talent.
