We are seeking a Board Eligible orBoard Certified Family Medicine or Internal Medicine Physician to become partof a collaborative care team focused on improving health outcomes throughpreventive care, chronic disease management, and coordinated treatment. Practice alongsideexperienced Nurse Practitioners, Care Managers, Pharmacists, Social Workers,and Clinical Support Teams in a supportive environment designed to helpphysicians focus on delivering exceptional patient care. Diagnose and manage acute,chronic, and complex medical conditions. Interpret diagnostic testsand coordinate specialty referrals when appropriate. Collaborate withinterdisciplinary care teams to optimize patient outcomes. Promote preventive care,wellness, and chronic disease management. Participate in qualityimprovement initiatives and patient engagement programs. Maintain accurate and timelyelectronic medical record documentation. Qualifications: MD or DO from an accreditedmedical school Active Arizona medicallicense (or eligibility to obtain) Current DEA registration Board Certified or BoardEligible in Family Medicine or Internal Medicine Passion for deliveringvalue-based, patient-centered care Excellent communication andcollaborative care skills
