As the Resident Advocacy Director (RAD) at our Assisted Living community in the Midlands of South Carolina, you will serve as a key support in resident care, assisting with clinical coordination, care management, family communication, and Navion Cares implementation to ensure continuity and quality of services.

You will spearhead enrollment in Medicare care coordination programs, develop and implement personalized care strategies, and ensure cohesive collaboration across interdisciplinary teams and external partners. Additionally, using your unique training and experience, you will provide vitally necessary behavioral health services to community residents.
